By submitting this Training Application, I am requesting that the staff of Bayside Bed & Biscuit evaluate and determine, in its sole judgment, whether my pet may be suitable for participating in the “Training Program” and/or social programs hosted by Bayside Bed & Biscuit. I understand and hereby agree that:

  1. I recognize that there is an inherent risk of injury, illness or death in any environment associated with numerous  pets. I also recognize that such risks include, without limitation:  
  • Injuries, usually benign, such as broken nails, sore paw pads, puncture wounds, abrasions and cuts,  particularly in shorter coated breeds, etc.  
  • Transfer of a communicable illness such as “canine cough” also known as Bordetella Virus, or “puppy  warts” also known as the Canine Papilloma Virus.  
  • Illness or death resulting from stress, altercations, rough play, or contagious diseases.  
  1. I understand that Bayside Bed and Biscuit cannot be held responsible for any injury, illness, death or damage  caused by my pet and that I am solely responsible.  
  2. I understand that I am solely responsible for the behavior and health of my pet participating in the Training  Program and related activities. 
  3. I agree to be responsible for my conduct and the conduct of my dog and others accompanying me (including  minor children) at all times.  
  4. I understand that I am solely responsible for any harm or injury caused by my pet to human(s) or animal(s) and I  expressly agree to be held responsible for medical costs due to injuries caused by my pet.  
  5. I am fully aware of the “group activities” fees and agree to pay in full as agreed.  
  6. I represent that my pet is:  
  • In all respects healthy and has received all required vaccinations.  
  • Does not currently have and has not had within the prior 30 days, any communicable diseases.  • Is on flea and tick preventative.  
  • Does not suffer from any disability, illness, or condition which could affect it, other pets, or employee’s  safety at Bayside Bed and Biscuit.  
  1. I understand and agree that if any problem develops while my dog is on the Bayside Bed and Biscuit campus, my  pet will be treated as deemed best by Bayside Bed and Biscuit’s owner, employees, and agents in their sole  discretion, and that I assume full financial responsibility for any and all expenses involved.  
  2. I agree to provide proof of required vaccinations for each dog prior to vaccinations expiring.  
  3. I expressly agree to be held responsible for any damage to property (i.e., lodging area, fencing, walls, flooring,  etc.) or other costs incurred by my pet.  
  4. I understand that if my dog displays aggressive behavior, that for the safety and health of my dog and others,  he/she will be confined to an enclosure.  
  5. I understand that “pet” may refer to one or more than one pet. 

I REPRESENT THAT I HAVE MADE FULL DISCLOSURE AND HAVE READ, UNDERSTAND, AND ACCEPT THE  TERMS AND CONDITIONS STATED IN THIS AGREEMENT, AND ACKNOWLEDGE THAT THIS AGREEMENT  SHALL BE EFFECTIVE AND BINDING UPON THE PARTIES AND THEIR RESPECTIVE SUCCESSORS AND  ASSIGNS.

Board and Train

 

The trainer will work with your dog for a minimum of two sessions per training day.

• The owner understands that during the Board & Train the trainer may have days off work. The total amount of training time will always be such that the trainer keeps the dog on track to achieving goals.

• If the trainer deems it appropriate, your dog will attend group play or individual playtime in addition to training. These will also occur on the trainer's days off.

• The trainer will give periodic progress reports to the owner.

• If your dog has not met some goals, the trainer will also review whether these goals can be met with additional training (additional
cost may be required) or with a private lesson at home.

• Drop off is between 8 am - 9 am on the day scheduled with the trainer.

• Pick-up time on your dog's last day will be scheduled between you and the trainer. Please allow 30 minutes - 1 hour to work with the trainer on the training to be implemented at home to maintain training goals.

• It’s the owner's responsibility to set up the included 1 private lesson within 60 days of the last Board and Train session. After the 60 days have passed it will be at the owner's expense to pay for a private lesson if desired.

• The owner's dog must have all required vaccinations before attending the first training day.

• The trainer will be available for phone calls and emails at no charge for 60 days after the last Board and Train day to help the owner maintain training.

• Any dog deemed to have fur/hair that could get matted is required to be brushed every 2-3 day. This will incur an additional fee.

Day Training

• The trainer will work with your dog for a minimum of two sessions each day.

• It is required to commit to at least 5 consecutive days of Day Training. The first four days of Day Training are held Monday-Thursday, and the 5th day of Day Training will be held the following Monday. Unless
otherwise scheduled with the trainer.

• Monday-Wednesday you will be sent home with a report card on what was covered that day. On Thursday, arrive before 5 pm to allow 15-30 minutes to work with the trainer on the skills that were covered, also on Thursday you will go over homework for the weekend.

• Drop off is between 8 am - 9 am and pick up is between 4 pm - 5 pm, unless arranged with the trainer differently. Picking up your dog early may result in them not receiving the full benefit of the session.

• The owner's dog must have all required vaccinations before attending
the first training day.

• The trainer will be available for phone calls and emails at no charge for 60 days after the last day train day to help the owner maintain training.

Maintenance - Board and Train, Day Training

The owner understands that the training will result in the dog responding to the trainer and that the dog may not initially respond as reliably to the owner. To ensure the dog’s response to the owner, the owner agrees and is responsible:

• To understand and apply the basic principles of training as described and demonstrated by the trainer.

• To use the cues supplied by the trainer.

• To practice with your dog on a daily basis, 5-10 minutes per day.

• To integrate training into daily life so as to
support behavior goals (not inadvertently punishing the good behavior and rewarding the bad.)

• To follow training and management advice as given by the trainer classes

• Class is set up so the trainer teaches you how to train your dog.

• The trainer will do a demonstration of the skill with a demo dog or one of the student's dogs. Then the owner will practice the skill in class with the trainer's help.

• It is the responsibility of the owner to be on time and present for each class.

• Make-up classes are not available for missed classes; however, an abbreviated lesson before the next class you attend may be available.

• It's the owner's responsibility to follow through with the homework and practice with your dog on a daily basis, 5-10 minutes per day.

• Children under 12 may attend class with adult supervision but may not handle the dog during training class. Children may be at an increased risk of injury, and may be a distraction to the owner and class.

• The owner's dog must have all required vaccinations no later then the second class. Or they will not be able to attend. No refund will be issued.

• The trainer will be available for phone calls and emails at no charge for 60 days after the last class day to help the owner maintain training.

Private Lessons

• A Private Lesson consists of working one on one with a trainer for 1 hour.

• The location will be at your home (if within a reasonable distance), at a neutral location (park), or at Bayside Bed & Biscuit.

• The trainer will do a demonstration of the skill with your dog. Then the owner will practice the skill with the trainer's help.

• It's the owner's responsibility to follow through with the homework and practice with your dog on a daily basis, 5-10 minutes per day.

• If a 3 lesson package is purchased, it's the owner's responsibility to schedule lessons with the trainer before 6 months' time has passed after the purchase date. After 6 months have passed it will be at the owner's expense to pay for a private lesson(s) if desired.

• The trainer will be available for phone calls and emails at no charge for 60 days after the last lesson day to help the owner maintain training. Payment, Reschedule, Refund and Cancellation

Please see the specific guidelines for each service listed below.

Board and Train

Payment
• A deposit of $200 is due when training is booked.
• 2 weeks - $1,200 total. $200 deposit upon booking, $400 due the first day, and $600 due final day.
• 3 weeks - $1,800 total. $200 deposit upon booking, $700 due the first day, and $900 due final day.
• 4 week - $2,400 total. $200 deposit upon booking, $1,000 due first day, and $1,200 due final day.
• Additional weeks - $600 each week.
• Also, a $10 payment is due the first day to cover the Healthy Pet Guarantee.

Reschedule
• Up to 14 days prior to start date, deposit can be used as a credit for a rescheduled training appointment. Reschedule must be
done within 30 days of the first scheduled training start date. It’s the owner's responsibility to reschedule within 30 days of the original start date. After the 30 days have passed a new deposit is required to reschedule.

• After 14 days prior to start date, deposit is forfeited, a new deposit is required to reschedule

Refund
• Up to 14 days prior to start date, deposit can be refunded 90% back.
• After 14 days prior to start date, deposit is non-refundable.

Cancellation

• Up to 14 days prior to start date, deposit is refunded 90% back.
• After 14 days prior to start date, deposit is non-refundable.
• After the first training session, payment is non-refundable.

Day Training

Payment
• A deposit of $200 is due when training is booked. The rest of the payment is due the first day.
• 5 days - $500 total. $200 deposit upon booking and $300 due the first day.
• 10 days - $1,000 total. $200 deposit upon booking and $800 due the first day.

Reschedule

Classes

Payment
• Payment is due before the first class to secure your spot, unless otherwise discussed with a trainer.
• 6-week class cost $200

Reschedule
• Before the first training session, reschedule must be done within 6 months of the first scheduled class start date. It’s the owner's
responsibility to reschedule within 6 months of the original start date. After 6 months have passed a new payment is required to
reschedule.
• After the first training session, reschedule must be done within 6 months of the first scheduled class start date. It’s the owner's
responsibility to reschedule within 6 months of the original start date. After 6 months have passed a new payment is required to reschedule.

Refund
• Up to 48 hours prior to start date, payment can be refunded 90% back.
• After 48 hours prior to start date, payment is non-refundable.

Cancellation
• Up to 48 hours prior to start date, payment is refunded 90% back.
• After 48 hours prior to start date, payment is non-refundable.
• After the first training session, payment is non-refundable.

Private Lessons

Payment
• Payment is due before the day of a private lesson, unless otherwise discussed with a trainer.
• 1 lesson - $120, 3 lesson package - $345

Reschedule

• Up to 48 hours prior to start date, reschedule must be done within 30 days of the first scheduled training start date. It’s the
owner's responsibility to reschedule within 30 days of the original start date. After the 30 days have passed a new payment is required to reschedule.
• After 48 hours to start date, payment for 1 Private Lesson is forfeited, a new payment is required to reschedule, or if purchased a 3 lesson package, only one lesson is forfeited.

Refund

• Up to 48 hours prior to start date, payment can be refunded 90% back.
• After 48 hours prior to start date, payment is non-refundable.
• Not showing up to appointment, payment is non-refundable.

Cancellation
• Up to 48 hours prior to start date, payment is refunded 90% back.
• After 48 hours prior to start date, payment is non-refundable.

Other exceptions

• Reschedule - No additional payment will be due under these circumstances as long as the rescheduling is done within 6 months of
the first scheduled training day. It's the owner's responsibility to reschedule with the trainer.
• If at any time during the training, an intact female is in or starts heat, the training session will end and the client will reschedule after the dog has finished its heat cycle. We require 1 week after the last heat cycle day before you can start or resume training.
• If at any time before or during the training the dog becomes heartworm positive, the training session will end and the client
will reschedule after the dog has received a negative heartworm test.
• If at any time before or during the training the dog becomes ill, the training session will end and the client will reschedule after
the dog has received a clean bill of health from their vet.
• The trainer reserves the right to reschedule training sessions due to changes in the trainer's schedule, unforeseen circumstances, or emergencies. The owner will be notified via phone call or email, and the trainer will reschedule with the owner.

Cancellations
• If at any time during the sessions the trainer feels the owner is not reinforcing the behaviors learned, the trainer has the right to
cancel the session(s).
• If at any time during the training, the trainer feels the dog is exhibiting aggressive, fearful, compulsive, anxious, phobic, or any behaviors that interfere with the training methods or endanger the health and safety of any participants or bystanders, including the dog, the trainer has the right to cancel
the session(s).
• If the trainer feels that the owner has motives, goals, or demands that are harmful or unethical, the session(s) will be canceled.

Photo & Video Release
Owner hereby grants Bayside Bed & Biscuit the absolute right and permission to copyright, use, re-use, publish and republish videos, photographic portraits, or pictures of my pets (listed above) taken during animal behavior consultations, training sessions, and in relation to other animal-behavior contexts.

The photos and videos may be used for advertising, marketing, educational,
trade, or any other purposes whatsoever.

Walking Equipment
• The owner understands that their dog must be wearing a safe and
secure collar and leash unless otherwise instructed by the trainer. The
collar and leash must not be weathered or tearing. And the dog must
not be able to slip out of the collar.
• No retractable leashes are permitted.

Health
The owner represents that their pet is:
• In all respects healthy and has received all required vaccinations. I agree to provide proof of required vaccinations for my dog prior to the vaccination expiring.
• Required Vaccinations: The following vaccinations are required and must be maintained current after the initial series of puppy boosters: Rabies, DHPP, Bordetella, and CIV. All vaccinations must be administered by a licensed, accredited vet.
• All required vaccinations must be received prior to training start
day.
• In good health and has not been ill with any communicable disease in the last 30 days. I agree to inform Bayside Bed & Biscuit of any such communicable disease that my pet may contract in the future.
• On monthly flea, tick, and heartworm preventative.
• Not suffering from any disability, illness, or condition which could affect it, other pets, or employee’s safety at Bayside Bed & Biscuit.
• Not done harm, or shown aggression or threatening behaviors towards any person, dog or any other animals. I have disclosed this in the Training Questionnaire. I understand that I must disclose to Bayside
Bed & Biscuit if my dog has bitten a person or had any aggressive behavior displayed in the past, current, and future.
